How to know when to use 'usted' in Spanish?

Always use "usted" for business and in formal situations. When you first meet people outta business situations I think "tu" is better if you want to sound approachable and friendly. Anyway intuition can help a lot. There are countries where the formal treatment is very common and others where it is reserved for very formal and business matters.
